The Right to Choose

Requesting Services
Receiving hospice services begins with a request for those services. This may come from a doctor, hospital discharge planner, nurse, a family member or directly from the patient. This process begins by calling our intake number, local office or contacting any of the wonderful staff from one of our locations. This is known as a referral to “evaluate and admit to hospice services if appropriate.”
What is the evaluation like?
After receiving a request to evaluate, we contact the patient and family within the hour to set up a face-to-face meeting:
• The nurse will ask a few questions to find out if you are appropriate for hospice services.
• A nurse will explain hospice services in detail to make sure that they meet your needs and wishes.
• If you are admitted into hospice care, you will be able to choose which services you wish to receive.
• A social worker will visit you to find out if you and your family have other needs we can meet
What services can I choose?
• Spiritual care provided by a chaplain
• Help with daily living activities such as bathing, cleaning and meals
• Volunteers to spend time with you, assist your family, run errands and sit with you so family can rest
• Family support services to help get your affairs in order and plan for the difficult days ahead
• Grief support services are available to your family, friends and caregivers
What happens after admission?
After admission, necessary medical equipment may be delivered to the home. Our wonderful nurses will then develop an individual plan of care based on your needs and diagnosis. You can make your own health care choices and we are here to serve you and your family. Your schedule of visits will be determined by your plan of care and your needs. In addition, we are on call 24 hours a day, 7 days a week if you need us. Every patient call initiates a face-to face visit.
Our goal is to make you comfortable so that you can live your life and we help you prepare for the challenging days ahead. Everything we do is meant to help meet that goal.
