Is Your Patient Ready for Hospice?
  • Does the patient have a life-limiting diagnosis?
  • Is the patient elderly and frail with other progressive medical problems?
  • Is the disease progressing despite maximal treatment?
  • Does the patient have cancer, end-stage congestive heart failure, end-stage pulmonary disease or other terminal diagnosis?
  • Is the patient experiencing shortness of breath that limits function?
  • Does the patient have frequent hospitalization recurring infections or unscheduled office visits?
  • Has the patient’s function or nutritional status declined in the past 30 days?
  • Would you not be surprised if the patient died in the next 6 months?
If you answered yes to any of these questions, we may be able to help your patient. Hospice makes the most positive impact on patients when care is received early in the diagnosis. If you have a patient you think is right for hospice, we can evaluate them to see if they qualify to receive care.
